Only patients with one of the following medical conditions may currently participate in Ohio’s medical marijuana program: AIDS, amyotrophic lateral sclerosis, Alzheimer’s disease, cancer, chronic traumatic encephalopathy, Crohn’s disease, epilepsy or another seizure disorder, fibromyalgia, glaucoma, hepatitis C, inflammatory bowel disease, multiple sclerosis, pain that is either chronic and severe or intractable, Parkinson’s disease, positive status for HIV, post-traumatic stress disorder, sickle cell anemia, spinal cord disease or injury, Tourette’s syndrome, traumatic brain injury, and ulcerative colitis. The first step is to have your primary care physician send your medical records to us for review. Submit the Authorization for Release of Medical Records form to your physician.
We cannot complete your evaluation without your medical records, please ensure you can provide them prior to your appointment.
Use our patient portal to register as a patient.
Once we receive your medical records and your $50 deposit, we will get you scheduled to see our physician, Sean Stiltner, D.O.
Please bring a state issued photo ID and a way to access your email so that we can assist you in completing your Ohio Medical Marijuana card registration, if you are qualified.
New patients: $150
Returning patients: $100
The $50 deposit will be applied to the total visit cost.
