To have your records transmitted to our office, submit the Authorization for Release of Information form to your primary care physicians office. The form contains all the info you and your physician will need to transmit your relevant medical records.
You can also upload your medical records directly to us using our Patient Portal.
As long as you let our office know that you cannot make your appointment time 24 hours in advance, you will not lose your deposit.
No, we cannot pursue your records for you. Your physician will require your signed consent to release your records to us, please use our Authorization for Release of Information form to have your records transmitted to our office.
Currently, we can only accept cash and credit card payments.
Checks and insurance payments cannot be accepted.
Our physician cannot make a recommendation without your relevant medical records. Please confirm with our office 24 hours prior to your appointment time if you are expecting your records to be faxed or emailed to our office to ensure you will not lose your deposit.
If you do not supply medical records for your appointment, we cannot make a recommendation and you will lose your deposit. We encourage you to use our Authorization for Release of Information form to retrieve the proper medical records.
